Hospitals Cedars-Sinai Medical Center is an 886-bed non-profit,
acute, quaternary, teaching Level I Trauma Center hospital located
in the heart of Los Angeles, California. The Department of Pharmacy
has over approximately 430 staff members who provide comprehensive
inpatient pharmacy services as well as outpatient services via 2
cancer centers and 3 outpatient pharmacies. We are committed to the
education and training of pharmacists, students/interns, residents,
technicians, and other health care professionals. We are affiliated
with several Schools of Pharmacy and offers PGY1 and PGY2 residency
programs. The residency and student experiential programs at CSMC
continue to grow with every successive year, where nearly 100
residents have graduated from the PGY1 pharmacy residency program
since 1985 and more than 200 student rotations are offered at CSMC
annually. Recently, CSMC was recognized and bestowed ---Site of the
Year--- awards by two major affiliated CA Schools of Pharmacy for
providing an exceptional experiential learning environment for
their students. The Pharmacotherapy Clinic Pharmacist is
responsible for providing comprehensive medication management for
patients with conditions requiring specialty medications upon
receipt of a provider referral and in accordance with Pharmacy &
Therapeutics (P&T) approved policies. The pharmacist is
responsible for ensuring required tasks are performed to maintain
specialty pharmacy accreditation(s). The pharmacist performs all
job duties in accordance with pharmacy policies, procedures,
regulatory requirements, best practice documents, and standard
operating procedures. The pharmacist performs these duties in a
timely, accurately, and appropriately prioritized manner to meet or
exceed customer expectations while exhibiting behavior that
supports Cedar-Sinai---s mission, vision, and values. The
pharmacist takes initiative to continually learn and maintain
clinical and regulatory knowledge. Demonstrates individual
commitment to cooperation and collaboration among staff and
customers. Perform routine patient assessments including gathering
relevant patient history to manage specialty disease states
Demonstrate clinical knowledge to initiate, adjust, or discontinue
patient medication regimen including but not limited to specialty
and adjunct medications, per P&T approved policies Order and
interpret drug therapy-related laboratory tests Perform follow-up
monitoring to assess and manage specialty medication side effects
or other symptoms associated with the patient---s underlying
conditions, drug-drug interactions, medication adherence, and any
relevant clinical laboratory results to achieve medication therapy
goals Provide medication education to patients and/or family
members. Accurately documents all patient and prescriber
communications in CS-Link Interpret and fill prescriptions in an
accurate and timely manner, according to policies and procedures,
while prioritizing workflow and maintaining required documentation
Refer patients to other healthcare providers/facilities, agencies,
and community resources. Perform required tasks to maintain
specialty pharmacy accreditations(s) including, but not limited to
prescription billing, prior authorizations, patient financial
assistance, medication reconciliation, patient
counseling/education, prescription refill reassessments, inventory
management, and mail order Use technology safely and effectively to
deliver medications and services to patients Supervise and train
technicians, pharmacy residents and/or students, including
supervision of daily workflow Demonstrates knowledge of third
party, Medi-Cal, Medicare Part B, and interdepartmental charging
procedures Communicate with patients, providers, and team members
(written, verbal) in an effective and timely manner Documents
clinical interventions, including near miss prescribing errors and
recommendations, for safe and effective medication management.
Oversee all dispensing and distribution processes within the
pharmacy according to state and federal regulations Actively
identifies methods and approaches to simplify work processes,
decrease medication wastage, and increase the quality of services
provided Performs other duties as assigned
