Job Description Job Description Reno Staffing is looking for Licensed Physical Therapists for a Home Health Agency in the Reno/Sparks and local area! Shift: Varied - 1 to 2 times per week Pay: $95 per Visit Location: Reno, Sparks and local areas. Summary: A Home Health Physical Therapist assesses, plans, and provides skilled therapy in patients' homes to improve mobility, manage pain, and restore function, focusing on individualized plans, family education, home safety, and collaboration with healthcare teams, requiring independent work, documentation, and a valid PT license. Key duties include evaluations, developing care plans, manual therapy, patient/family training on exercises, home safety assessments, and maintaining compliance with regulations for quality patient outcomes. Essential Duties and Responsibilities: Other duties may be assigned. Patient Assessment & Planning: Conduct comprehensive evaluations to identify functional limitations, develop individualized treatment plans, and set measurable goals. Treatment Delivery: Provide skilled physical therapy (manual therapy, exercises, modalities) to improve strength, balance, mobility, and pain management. Patient & Family Education: Teach patients and caregivers therapeutic exercises, home programs, and strategies for safety and independence. Home Safety: Perform home safety assessments and recommend necessary modifications. Interdisciplinary Collaboration: Work closely with physicians, nurses, and other team members for coordinated care. Documentation: Maintain accurate, timely, and compliant records of visits, progress, and changes in condition. Communication: Report patient progress to the care team and participate in case conferences. At the discretion of your lead or supervisor, you could be required to perform additional job related activities not specifically outlined above. Essential Skills & Requirements Licensure: Valid Nevada Physical Therapist (PT) license. Experience: 1 years of home health or similar experience, strong clinical skills, and problem-solving abilities. Independence: Ability to work independently, manage a flexible caseload, and adapt to changing patient needs. Interpersonal Skills: Compassionate, excellent communication, and ability to build rapport. Logistics: Reliable transportation and valid driver's license. Must be able to pass urine drug test and background check. Current CPR/BLS Certification Bilingual is a plus! Typical Work Environment Delivering care in patients' homes, promoting independence and quality of life within their familiar surroundings.
