Job Description:
A company is looking for a Care Coordinator to provide care
coordination for Medicaid, Medicare Advantage, and Dual Eligible
Special Needs Plan members.
Key Responsibilities
Conduct outreach to educate members about program benefits and
facilitate care coordination
Assess member status and develop personalized care plans to address
their goals
Manage a caseload, ensuring timely contact and coordination of care
with healthcare providers
Required Qualifications
2+ years of experience in care coordination, case management, or
related fields
Experience working with Medicaid and/or Dual Eligible patients
Remote work experience preferred
Experience with documenting case notes in care management or
electronic health record platforms
Dedicated home office setup for remote work
